In 4 weeks, all of your paperwork has to be turned in! Let's not put this off! The longer you wait to do it the more stress you are going to be putting on yourself. If we don't have all of these documents turned in to us you will NOT be able to go on the trip! Please call the office if you have any questions about the paperwork that needs to be turned in.

If you do not get your paperwork in it could disqualify you from going on the trip. Make sure you have your Doctor's appointments made today!

Here is the paperwork that needs to be turned in:

Copy of Passport: If not already turned in

Medical Release: This needs to have a Doctors Signature

Participants Release:

This needs to be notarized

Shot Record: This MUST be on the form we have provided

Proof of Insurance: Copy the front and back of your Insurance Card
